The new General Data Protection Regulation laws came into force on 25 May 2018 and determine how your personal data is processed and stored and your legal rights in relation to your personal data.

The Clinic of well-being takes your confidentiality and privacy rights very seriously and how your data is collected, processed and securely stored forms part of our accountability and transparency policy to you under the GDPR regulations 2018.

What information do we collect from you?

Records about your health and the treatment/therapy you receive.

Basic details such as name, address, date of birth, phone number and e-mail

Notes and reports about your physical or mental health and any treatment you may need or receive

Relevant information from your GP or other health care professional, family member or carer.

Records of appointments at the clinic and outcome of the therapy received

Copies of any correspondence in relationship to employers or personal insurance or third party

Your feedback and the outcome of any treatment, service, therapy that you have received

Any information stored under your client records will only be used to benefit your care and personal information will never be shared with anyone or any company, establishment, member of the public, agency, insurance company, marketing company or individual under any circumstances unless ordered to do so by a court of law or requested by another healthcare professional if our duty of care deems this to be in the best interest of your health or well-being.

Why do we collect information?

Your information is used to guide and record the care you receive and assessing your further needs. It is vital with any care/therapy to have all information that relates to present and ongoing care and your future needs. We also need to store your contact details to arrange future appointments or in case we need to change/cancel such appointments and also to satisfy the requirements of our professional bodies and data retention guide lines set out by insurance requirements and government agencies

We are required by law to retain your information for a period of at least 8 years as specified by our professional bodies and our insurance suppliers and the Inland Revenue guide lines.

This is also known as the right to be forgotten.
